Patient requires ambulance transport due to REPLACE ME (specific functional limitation preventing safe ambulation or safe travel by POV/wheelchair van).
Transport by other means was unsafe because REPLACE ME (severe weakness, inability to stand or pivot, fall risk, oxygen requirement, confusion, post-procedure condition, pain, non-weight-bearing status, etc.).

Dispatch

ProMed Ambulance Company Unit Unit Number was dispatched non-emergent to Pick-Up Location / Facility for a patient requiring ambulance transportation for discharge following treatment.
Unit responded without lights or sirens with Crew Members.

Complaint

Patient was evaluated and treated at the facility for REPLACE ME (reason for admission or ED visit).
Today the patient is being discharged but remains unable to safely ambulate or self-transfer due to REPLACE ME (current condition).

History

Past Medical History: REPLACE ME.
Allergies: REPLACE ME or NKDA.
Nursing staff report REPLACE ME (course of treatment, changes from baseline, mobility status, abnormal labs, pain, ongoing symptoms, any precautions or restrictions).

Assessment

Patient found in REPLACE ME (room + location) lying REPLACE ME (position).
Mental status: REPLACE ME (A&Ox#, confused, lethargic, etc.).
Airway/Breathing: REPLACE ME (normal/labored, oxygen requirement).
Circulation/Skin: REPLACE ME.
Mobility: Patient is unable to ambulate / unsafe to pivot / high fall risk / unable to bear weight / weak / painful / requires two-person assist due to REPLACE ME.
Vitals: REPLACE ME.
Notable exam findings: REPLACE ME.

Vital signs were obtained, documented, and monitored throughout transport.

Patient required continuous monitoring during discharge transport due to REPLACE ME (weakness, oxygen needs, pain, instability, altered mental status, etc.).

Transport

Patient was found in REPLACE ME (room/bed location) in Semi-Fowler’s / supine / other position.
Patient was REPLACE ME (sheet-lifted, stand-pivot with assistance, 2–3 person assist) onto the stretcher due to functional limitation and secured with all straps and rails.

Patient was transported into the ambulance, loaded, and secured.
Unit then went enroute to Destination Name or Address.
Patient was monitored throughout transport and REPLACE ME (remained stable / mild changes).

Upon arrival, patient was transported into Residence / Facility Name and moved safely into bed/chair using a draw sheet / pivot assist.
Care turned over to REPLACE ME (family/caregiver/staff).
Unit cleared and returned in service.
