The patient requires ambulance transport due to REPLACE ME (specific functional limitation or clinical condition) preventing safe travel by POV/wheelchair van.
Medical necessity today is supported by REPLACE ME (e.g., weakness, severe edema, SOB, fluid overload, dizziness, hypertension, AMS, inability to ambulate, oxygen requirement).

Dispatch
ProMed Ambulance Company Unit Unit Number was dispatched to Pick-Up Location / Address for a patient requiring non-emergent ambulance transportation for a scheduled dialysis treatment.
Unit responded without lights or sirens with: Crew Members.

Complaint
The patient is being transported for evaluation and treatment of end-stage renal disease requiring ongoing dialysis.
Today the patient presents with REPLACE ME (continued swelling, fluid overload, abnormal weight gain, anuria, SOB, weakness, poor mobility, dizziness, fatigue, hypertension, etc.).

Past Medical History: REPLACE ME.
Allergies: REPLACE ME or NKDA.
Staff/family report REPLACE ME (recent missed treatments, worsening symptoms, difficulty walking, baseline mobility issues, etc.).

Assessment
REPLACE ME (overall appearance, position found, distress level).
Mental status: REPLACE ME (A&Ox#, confused, fatigued).
Airway/Breathing: REPLACE ME (normal/labored, SOB, oxygen level needs).
Circulation/Skin: REPLACE ME (edema, skin signs, perfusion).
Mobility: Patient is unable to ambulate / unsafe to pivot / high fall risk / weak / unsteady due to REPLACE ME.
Weight trends if reported: REPLACE ME (e.g., +5 lb fluid gain).
Vitals: REPLACE ME.
Abnormal findings: REPLACE ME.

Vitals were obtained, documented, and monitored throughout transport.

Patient secured with all straps and rails.
Patient loaded into the ambulance and secured.
Unit went enroute to Destination Name or Address.
Patient monitored throughout transport and REPLACE ME (remained stable / showed mild symptoms / etc.).
On arrival, patient was transported into Dialysis Facility Name and moved safely to the dialysis treatment chair using REPLACE ME (sheet lift, assist, etc.).
Care was transferred to Receiving Staff Name.
Unit was clear and back in service.
